Appointments must be scheduled at least 48 hours in advance. Cancellations or rescheduling requests should be made at least 24 hours before the scheduled appointment. A cancellation fee of $50 may apply if you fail to cancel or reschedule within the required time. No-shows without prior notice may incur a charge equivalent to the full cost of the missed appointment.
Payment is due at the time of service unless prior arrangements have been made. We accept cash, credit/debit cards, and most insurance plans. Patients are responsible for any co-pays, deductibles, or non-covered services.
By scheduling an appointment, you consent to the treatment options explained by our dental team. All treatment will be performed with your consent and understanding, following HIPAA guidelines.
Your personal and medical information is confidential and will not be shared with third parties unless required by law or with your explicit written consent.
Patients are responsible for verifying insurance coverage and paying any remaining balances after insurance claims.
If you arrive more than 15 minutes late, your appointment may need to be rescheduled.
Respectful behavior is expected. Inappropriate conduct may result in termination of the patient-provider relationship.

Emergency care is provided during business hours. For after-hours emergencies, please contact your nearest provider.

Patients have the right to access and amend health records. Complaints can be filed with Connect Family Dental or HHS.
